As a General Practitioner or Health Professional, you can earn points toward your ongoing professional development by participating in a ThinkGP educational activity that will support you to encourage healthy lifestyle changes in your patients.

This ThinkGP module, Helping our Patients to Get Healthy is about how you can help your patients by facilitating their access to the NSW Ministry of Health’s free Get Healthy Information and Coaching Service, within an overview of eligibility, the referral process and follow ups.

By completing this learning activity, participants will be able to:

  • Understand the role of the Get Healthy Information and Coaching Service within a multidisciplinary care program for patients
  • Discuss with patients their eligibility for the Service
  • Refer patients to the Service through appropriate means
  • Apply the follow-up reports received from the Service to improve quality of patient care.

This education activity is accredited: RACGP 2 CPD, ACRRM 1 CME PD.

Eligibility for CPD points vary between each professional body. Health practitioners are required to refer to their profession’s specific CPD requirements.
